Referrals
At Spectrum HC, we are committed to providing high-quality, personalised support to individuals in need, and we actively encourage referrals from health and social care professionals. Whether you are a professional seeking assistance for a client, a parent or carer looking for support for a loved one, or someone who is seeking help for yourself, we are here to offer the guidance and care you need.
Who We Support
Spectrum HC provides tailored support to:
- Individuals with a primary diagnosis of Learning Disabilities and/or Autism Spectrum Disorder
- People aged 16 and above
- Those residing in Surrey, London, Dartford, and Hampshire
What Happens When You Make a Referral?
When you make a referral to Spectrum HC, we will review it to determine if we have the capacity to offer support. If we are able to assist, the next step is to schedule an initial assessment.
During this assessment, which typically lasts 1-2 hours, we will meet with you and any relevant individuals to explore your needs and the type of support required.
We will discuss topics such as:
- Your diagnosis and medication
- The support you are looking for
This is also an opportunity for you to ask any questions.
After the assessment, we will decide if we are able to offer support and liaise with your social worker or case worker to move forward.
